Since Monday, the internet’s attention has focused squarely on 26-year-old Luigi Mangione, who has been arrested and charged with the murder of UnitedHealthcare CEO Brian Thompson in New York. The killing, which happened on December 4 at around 6:45 am, has unleashed a torrent of online reactions, with social media platforms flooded with memes, jokes, and heated debates. While major news outlets have been cautious in their coverage, independent reporter Ken Klippenstein leaked what appears to be Mangione’s manifesto, giving a clue into the alleged shooter’s motivations.
In the 262-word manifesto, Mangione explicitly criticizes the US healthcare system, drawing attention to how the US has the world’s most expensive healthcare system yet ranks around 42nd in life expectancy. He specifically called out UnitedHealthcare, noting it’s one of the largest companies in the U.S. by market capitalization.

My tech is pretty locked down because I work in engineering so probably not much info there. I do apologize for any strife of traumas but it had to be done. Frankly, these parasites simply had it coming. A reminder: the US has the #1 most expensive healthcare system in the world, yet we rank roughly #42 in life expectancy. United is the [indecipherable] largest company in the US by market cap, behind only Apple, Google, Walmart. It has grown and grown, but as our life expectancy? No the reality is, these [indecipherable] have simply gotten too powerful, and they continue to abuse our country for immense profit because the American public has allwed them to get away with it. Obviously the problem is more complex, but I do not have space, and frankly I do not pretend to be the most qualified person to lay out the full argument. But many have illuminated the corruption and greed (e.g.: Rosenthal, Moore), decades ago and the problems simply remain. It is not an issue of awareness at this point, but clearly power games at play. Evidently I am the first to face it with such brutal honesty.
Mangione’s personal background offers some explanation on why he might have been motivated to kill Thompson. Online posts reveal he struggled with significant back problems, experiencing firsthand the challenges of accessing medical care.
On Reddit’s spondylolisthesis forum, he once shared advice about persistently seeking medical treatment, writing, “Keep trying different surgeons. ‘Nobody will operate on my back until I’m at least 40’ is nonsense.”
To no one’s surprise, the internet’s reaction has been a mix of dark humor and commentary.
